| PURPOSE AND RATIONALE OF THE QUALIFICATION |
| The purpose of this Postgraduate Diploma in Clinical Forensic Medicine is to provide advanced education and specialised training in Clinical Forensic Medicine to suitably qualified medical practitioners, including medical officers, nurses, family physicians, and other relevant specialists working in Primary, Secondary, and Tertiary healthcare facilities. Through this qualification, learners will acquire advanced medico-legal knowledge, specialised forensic clinical skills, and professional competencies required to manage cases of forensic relevance.
The qualification is designed for practitioners who are involved in the assessment and management of victims and alleged perpetrators of violence, abuse, trauma, and other medico-legal conditions. Learners will benefit by enhancing their professional scope of practice, strengthening their competence in forensic assessment and documentation, improving their credibility within the justice system, and expanding career opportunities within both the health and medico-legal sectors. This qualification follows a professional and occupational pathway within the health sciences domain. On successful completion, the learner will demonstrate integrated theoretical knowledge and applied clinical competence in Clinical Forensic Medicine. The learner will understand medico-legal principles, relevant legislation, ethical frameworks, injury interpretation, evidence management, and court procedures, and will be able to apply these within complex clinical environments. Learners will be responsible for conducting comprehensive forensic clinical examinations, documenting findings in a legally defensible manner, preserving and managing forensic evidence, compiling medico-legal reports, and providing expert clinical input and testimony where required. The qualification, therefore, prepares learners for responsible professional practice within healthcare institutions and in collaboration with the justice system. Upon completion of this qualification, qualifying learners will be able to: In addition, learner attributes including the application of appropriate methods, procedures and techniques; the ability to access, process, analyse, manage and communicate information effectively; ethical reasoning and professional accountability; application of health and safety principles; problem-solving in complex contexts; and the exercise of independent judgement in line with relevant level descriptor i.e. NQF Level 8 will be integrated in the learning process to ensure holistic competence and responsible professional practice. Rationale: This qualification is justified by a clearly identified national need within the health and justice sectors. South Africa faces a sustained and complex burden of violence, trauma, and medico-legal casework that places significant pressure on the healthcare system, particularly at primary healthcare level where most assault-related and violence-related cases are first managed. High rates of physical and sexual assault, interpersonal violence, road traffic injuries, and alcohol-related offences require healthcare practitioners to possess advanced clinical forensic competence to ensure quality patient care and legally defensible documentation. National and international evidence further supports the need for this qualification. Data from the World Health Organization indicate that road traffic injuries remain a leading cause of death and disability globally, with the African region carrying the highest risk burden. In South Africa, practitioners are frequently required to conduct medico-legal examinations in cases involving driving under the influence of alcohol and other offences, complete statutory documentation, and provide expert testimony in court. Empirical studies have revealed significant gaps in practitioner knowledge and practice, including incomplete medical histories, inadequate examinations, and improper blood alcohol collection procedures that compromise the admissibility of evidence. These deficiencies are compounded by systemic challenges such as workforce shortages, inequitable distribution of professionals, high patient volumes, and limited access to structured training in Clinical Forensic Medicine. The rationale for this qualification is therefore grounded in the need to address clearly identified skills gaps and strengthen professional competence within regulated health professions. The qualification responds directly to labour market needs in the public health and justice sectors by developing advanced, integrated knowledge and applied competencies in Clinical Forensic Medicine. It aligns with national regulatory and ethical frameworks, including those of the Health Professions Council of South Africa and the South African Nursing Council, and may support formal recognition of additional competencies within scope of practice. By strengthening forensic capacity, the qualification enhances the credibility, reliability, and legal defensibility of medico-legal services. The typical learner entering this qualification will be a registered medical practitioner (such as a medical officer, family physician, or specialist) or a professional nurse with advanced clinical responsibilities in emergency, district, or primary healthcare settings. Learners will generally have prior clinical experience in the management of trauma, violence-related injuries, or medico-legal cases and will seek formalised, advanced training to enhance professional competence, ethical practice, and medico-legal credibility. The qualification supports career progression and professional development within regulated health professions and responds to the needs of practitioners operating in district hospitals, community health centres, emergency departments, forensic units, and related services. Learners of this qualification will typically function as autonomous professionals in roles such as Medical Officer, Family Physician, Emergency Medicine Practitioner, Forensic Medical Practitioner, Clinical Forensic Examiner, Sexual Assault Care Practitioner, Medico-Legal Consultant, or Expert Witness in criminal and civil proceedings. They will operate across primary healthcare facilities, district and regional hospitals, tertiary academic hospitals, sexual assault care centres, medico-legal services, private practice, and courts of law. By improving the quality of forensic medical examinations, documentation, and expert testimony, this qualification contributes directly to improved service delivery, strengthened medico-legal systems, protection of vulnerable populations, and the promotion of constitutional rights to dignity, equality, and access to justice within the South African context. INTEGRATED ASSESSMENT: The qualification aligns with the Assessment Policy at the institution and uses various types of assessment that enhance learner learning. Assessment adheres to the national requirements set by the HPCSA and CMSA as statutory professional bodies. Formative assessment: Continuous formative assessment through Work-integrated learning and Continuous Workplace Based Assessments (WBA): Direct observations, based on the prescribed syllabus, take place in clinical settings while the learner work with patients. Observers include the supervisor or consultants in related disciplines at the place of work as part of IPECP and can include consultants in the discipline or consultants in related disciplines, workplace supervisors, registrars, peers, nurses etc. Observers provide written feedback to the learner. Feedback is provided using approaches such as Mini-CEX, One-minute preceptor, SNAPSS, ISBAR etc. If necessary, a remedial plan for corrective action is developed. Skills and procedures are signed off by the supervisor once the learner achieved competence. A logbook with signatures, records of observation feedback and quarterly feedback meetings, and remedial plans forms part of the Portfolio of Evidence (PoE), as well as any other evidence of expected work (e.g., case-based presentations, participation in QI meetings, ward rounds where patients are being discussed etc).
